• A statement, under penalty of perjury, that the information provided is accurate
• Your physical or electronic signature

Counter-Notice

If your content was removed due to a DMCA notice, you may file a counter-notice including your contact details, identification of the removed content, and a statement of good faith belief that the removal was a mistake or misidentification.
